COVID-19 LIABILITY WAIVER

I/ We the undersigned, understand that attending Sassafras Earth Education Summer Camp 2023 carries certain risks associated with COVID-19. I/ We acknowledge that the risk of exposure to COVID-19 exists in any public place and that Sassafras Earth Education cannot guarantee that I/ We will not be exposed to COVID-19.

I/ We voluntarily assume the risk of exposure to COVID-19 and release and hold harmless Sassafras Earth Education, its owners, operators, employees, and subcontractors from any and all liability, claims, demands, damages, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me/ us or any of my/ our property, as a result of participating in the camp, whether caused by the negligence of the camp or otherwise.

I/ We agree that the participant is not experiencing any symptoms of COVID-19, such as fever, cough, shortness of breath, or loss of taste or smell, and that participant has not been in close contact with anyone who has been diagnosed with COVID-19 within the past 14 days.
